Japanese (people) are quite shallow in implementing other cultural elements. The fact Otonashi speaks and writes japanese is for the benefit of immersion for the (japanese) viewer.
However, in Belgium they speak dutch (/flemish) in the north of the country and french in the south. "in Belgian" or a belgian language does not exist.

Antwerpen Cafe Edit
I live close to Antwerp and the comment about language is indeed correct. However many elements like the buildings and rooms indicate that this part is in Japan. What would be special about cafes in Antwerp I do not know tho.
